1.Premature Ovarian Failure ( POF )

1. Amenorrhea lasting more than 4 months
2. Age less than 40 yrs, and
3. Serum Follicular stimulating hormone > 40 mIU/ml on two occasion

But there are reports that ,females with secondary amenorrhea may have intermittent ovarian function in 24% cases and pregnancy in 8% of cases.
Etiology of POF

Deficient initial follicle number
Accelerated follicular atresia
Autoimmunity

Enzyme deficiency
Autoimmunity
Lymphocytic oophoritis
Signal defects
Gene mutation
Iatrogenic
Idiopathic
Several autoimmune disorders have been associated with POF ;most common is hypothyroidism with incidence of 27 % ,followed by diabetes mellitus (2.5 % ) and Addison’s disease  ( 2.5 % )

Karyotype should be done in all cases if there is POF before 30 yrs. Y chromosome in any cell line necessitated gonadectomy to prevent gonadoblastoma
